Clinical interview
A structured diagnostic interview (DIVA-5) over secure video, exploring your experiences from childhood to today.

Questionnaires
Complete two validated measures online at your own pace: the Adult ADHD Self-Report Scale (ASRS) and the Executive Skills Questionnaire–Revised (ESQ-R).

Results & report
Receive your results and, if you wish, a written report to share with your primary care provider or psychiatrist.

Adults 18 and older in South Carolina who want to understand long-standing struggles with attention, focus, organization, or restlessness, whether you're just beginning to wonder or your doctor has suggested an evaluation.
For physicians
We welcome referrals from primary care practices. With your patient's written consent, we'll share the assessment report to support your treatment decisions. Call 803-386-9120 to set up a referral relationship.
